Permission to Publish

The Permission to Publish form must be completed by all authors of a paper submitted for review to the Journal. Each author must sign and date the form.

The form is available in Adobe Acrobat Portable Document Format (PDF). A free reader is available. The form can be filled out electronically, but must be printed, signed and faxed to +1 (757) 451-8156.
